Targeted Case Management (TCM)
Ensures that persons with mental health needs have the resources they need to enhance their ability to function independently in their community.
The Targeted Case Management Program provides case management services for adults in Calvert, Charles and St. Mary’s counties. TCM ensures that persons with mental health needs have the resources they need to enhance their ability to function independently in their community. TCM provides connections to resources and support to meet their basic needs.
Eligibility Requirements:
- Must have a mental health diagnosis
- Must be 18 years or older
- Must have or qualify for Medical Assistance (MA)
May have Medicare or be uninsured if criteria met: Homeless, Recently released from Jail or Prison, Recently Released from hospital for psychiatric admission.
Services provided include:
- Careful assessment of needs and future goals
- Connection to Mental Health Treatment
- Person-Centered service planning
- Assistance navigating complex benefit and entitlement systems
- Assessment of eligibility for the SOAR process to qualify for benefits
- Identifying and maintaining housing needs
- Connection to transportation assistance
- Assistance with landlords and housing programs
Case Management is designed to be a shorter-term service that provides a warm handoff to other appropriate services. Together, we create a unique plan of care centered on your unique strengths, needs, abilities and personal preferences.
