Psychiatric Rehabilitation Program (PRP)
Flexible, round-the-clock services are provided to individuals with a mental health diagnosis, centered around drop-in visits at home or on-site.
These services are offered on an individual, flexible, around the clock basis to persons with a mental health diagnosis. The services are person-centered and offered on a drop in basis in the person’s home, an on-site facility, or in a setting most conducive to promoting the participation of the individual in community life. Clients in the program receive at least 6 visits per month from their Rehab. Counselor, to assist the client to perform skills in their home, maintain their independence, and access community resources such as: psychiatric/therapeutic services, legal services, somatic care, financial aid, housing assistance, substance abuse services. The RC will coach the client through the process of locating, applying for and maintaining all entitlements. The client must maintain mental health treatment to apply and to remain in this program.
