Assertive Community Treatment (ACT)

Our Assertive Community Treatment (ACT) team uses a person centered approach to treatment planning and collaboration. Our team provides this active community service when and where it is needed.

ACT improves outcomes for people with severe mental illness who are most at risk for homelessness, psychiatric crisis and hospitalization and involvement in the criminal justice system. Our clinical outcomes show that once admitted to the team, clients decrease use of inpatient hospitalization by 46%, Emergency Department services by 52% and total medical expenses by 50% over the course of a year.

We offer a variety of supports and services including:

Housing Initiative Program

  • Service Coordinator for a group of clients who were previously homeless
  • Assist with housing over 30 clients since in past 3 years
  • Act as rep. payee for clients which assists client with financial and housing stability
  • Create partnerships for new housing options

Supportive Employment

  • Goal is to assist clients to obtain employment as part of recovery
  • Provide job coaching, interview support, benefit counseling, educational support and job development in the community

Co-Occurring Disorders

  • Utilize Stages of Change Model and motivational interviewing to assist clients in reducing Substance Use
  • Adhere to a harm reduction approach to intervention
  • Provide linkage to increased level of support as needed
  • Therapy /Social Work
  • Individual therapy
  • Group interventions
  • Family psycho-education
  • Support, counseling and community linkage as indicated

Nursing and Medication

  • Provide medication evaluation and complete medical & psychiatric history
  • Assist with med. management
  • Coordinate medical appointment, tests and labs
  • Communicate client needs to pharmacies and community providers
