Targeted Case Management program provides individualized, person-centered support for adults living with serious mental illness helping them navigate complex systems of care and access the resources they need to live independently in Calvert, Charles and St. Mary’s counties. Our trained case managers work directly with clients to assess their needs, develop individualized care plans, and coordinate essential services such as housing, healthcare, benefits, and community support.
This program serves as a lifeline for individuals at risk of homelessness, hospitalization, or incarceration by addressing barriers that often lead to crises such as income loss, unstable housing, or lack of access to treatment. Case managers link participants to vital services including behavioral health care, Medicaid enrollment, SSI and SNAP benefits, transportation, and vocational resources, ensuring that no one falls through the cracks.
Through regular check-ins, advocacy, and 24/7 crisis response, Cornerstone’s TCM team provides the consistent guidance clients need to achieve stability and recovery. Our approach is mobile, flexible, and deeply human meeting individuals in their homes, shelters, or wherever they feel safe. Every plan evolves with each participant’s progress, ensuring long-term support and accountability.

May have Medicare or be uninsured if criteria met: Homeless, Recently released from Jail or Prison, Recently Released from hospital for psychiatric admission.
Case Management is designed to be a shorter-term service that provides a warm handoff to other appropriate services. Together, we create a unique plan of care centered on your unique strengths, needs, abilities and personal preferences.
